Request a Professional Corporate/LLC Certificate

The formation of professional business entities by individuals licensed by the KSBHA requires certification by the KSBHA that: (1) each member or shareholder of the professional entity is licensed in the same profession stated in the professional of the articles of incorporation/organization; and (2) the business entity’s name is not contrary to law or the ethics of the professions involved.

To request a professional corporate/LLC certificate, please send an e-mail including the information described below to KSBHA_ProfCorpCerts@ks.gov

The email must include:

  1. The name of the corporation/LLC (including the designation of P.A., LLC, etc.)
  2. List of all owners, including:
    1. Name
    2. KSBHA license type
    3. KSBHA license number
  3. Professional purpose of the corporation/LLC (see below at “Request 1: Form a New Professional Entity”)
  4. The required completed forms for one of the four requests listed below
Request 1: Form a New Professional Entity
Request 2: Add a Shareholder/Member to an Existing Professional Entity
Request 3: Change a Professional Entity's Name
Request 4: Convert a General Business Entity to a Professional Business Entity

Failure to include each item necessary to issue a certificate will result in additional time for processing. The certificate will be emailed to the requestor unless otherwise requested.

Applications are processed in the order they are received and may take up to 30 days during peak filing times. Submitting an incomplete application or not being in compliance with the Kansas HealthCare Stabilization Fund (“KHCSF”) will put a hold on processing the application until the missing information is provided to the Board. If you do not receive an incomplete application notice or your Corporation Certificate within 30 days, please contact our office.
