Anyone who has knowledge of a licensee’s unprofessional conduct or inability to practice within the standard of care may file a complaint. Complaint forms are accepted from various sources including but not limited to patients, family members, public members, and healthcare organizations.

You do not have to fill out the complaint form. You can submit your complaint by regular mail, e-mail, or contacting the Complaint Coordinator by phone for assistance.

Please ensure you provide the following information: The licensee’s first name, last name, and practice address; your first name, last name, daytime phone number, e-mail address and residential address so we can contact you if more information is needed; the patient’s full name, date of birth, and a concise account of your concern related to the licensee listed on your complaint form along with dates of service or timeframe of occurrence.

If you have hypothetical questions that you would like for the Kansas State Board of Healing Arts’ General Counsel Department to address, please contact KSBHA.GC.HelpDesk@ks.gov.  You have the right to contact a private attorney. The Board cannot give legal advice, does not represent individuals and only has authority concerning the licensing of the licensee.

There is no statutory time limit in which to file a complaint. It is best to report as soon as possible so the records can be obtained, and potential witnesses can be located.  When submitting a complaint, be as specific as you can with the facts and dates.

Every complaint received is reviewed, even anonymous complaints.  While anyone can submit an anonymous complaint or request to remain anonymous, a person’s anonymity is not guaranteed as the information may become available through the discovery process.

Each licensee is required to maintain the patient record for a minimum of 10 years from the date the licensee provided the professional service recorded.  If the patient is a minor, the records are required to be maintained until the minor reaches their 19th birthday.  Any questions or concerns should be directed to:
